NATIONAL DONOR FAMILY QUILT
An Invitation to Donor Families

The National Donor Family Council invites you to participate in the making of the NATIONAL DONOR FAMILY QUILT. Dedicated to the memory of those who gave the Gift of Life, each quilt square incorporates pieces of memories into pieces of materials to commemorate the love and the life of your loved ones. The Quilt will be displayed nationally to help spread the message of organ and tissue donation, and to keep alive the memory of those who gave. In addition, families are encouraged to write stories (about 100 words) about the meaning of the quilt square's design and how you felt when you made it. These will be compiled into a book, "Stories from the Quilt."

Quilt squares should be 8" square, including a 1/2" margin - any color or fabric. Use crayons, paint, markers, thread or any other medium to design the square. Please feel free to include the name of your loved one and dates of birth and death, if you wish. Please try to ensure that the square will be dry cleanable.

Please send finished quilt squares to:
